If the main reason for your
examination is to update your
eyeglass or contact lens
prescription and you are not having
any other problems with your eyes,
then the exam is considered a
"routine" eye exam and is covered
under your vision plan, and not
your medical insurance.  Most vision
plans do not cover office visits
where medical issues are
addressed.
If you have a medical issue with your
eyes such as allergies, dry eyes,
floaters, glaucoma, cataracts, diabetes,
macular degeneration, trauma, iritis,
etc., your examination will be billed to
your medical insurance and not your
vision plan.  You can still use your vision
plan for eyeglass and contact lens
purchases.
